An Assessment of Resources to Support Transitioning Youth was developed in collaboration with FosterClub, a national network for youth in foster care, and the Fostering Connections Resource Center. This tool serves as companions to FosterClub’s Transition Planning Toolkit, a guide to help states implement the youth-directed transition plan mandates of the Fostering Connections Act. An Assessment of Resources helps state and community leaders identify areas in which additional services and supports need to be developed.

What I appreciate most about this FosterClub Transition toolkit is that each of the 10 domains has scenarios & examples based on hundreds of youth/alumni that FosterClub has directly worked with. Through the FosterClub All-Star internship: we have over 82 alumni of this internship (myself included) who have been able to contribute to this toolkit in hopes to help better serve and prepare the next generation of youth in and from foster care. http://allstars.fosterclub.com/
